Plating Smiles provides free, nutritious meal delivery services to eligible New Yorkers enrolled in a Medicaid Managed Care Plan who face food insecurity and belong to specific high-need groups. Qualified participants can receive up to 21 meals per week as part of the state's Medicaid waiver to address Health-Related Social Needs (HRSN).

2. Eligibility and Enrollment

• Eligibility: Access to our services is contingent upon verification of your eligibility under the New York State Medicaid SCN program.
• Assessment: To begin, you must complete an eligibility screening. We reserve the right to determine eligibility in coordination with your Medicaid Managed Care Plan.
• Accuracy of Information: You agree to provide accurate, current, and complete information during the enrollment process, including details regarding your health, Medicaid status, dietary restrictions, and delivery address.

3. Dietary Restrictions and Allergies

While we strive to accommodate specific dietary needs and food sensitivities, it is your responsibility to accurately report any allergies or restrictions during enrollment.

4. Delivery Policies

• We will deliver meals to the address provided during enrollment. It is your responsibility to ensure the delivery address is accurate and that someone is available to receive the meals or that a safe drop-off location is identified.
• We reserve the right to modify delivery schedules due to weather, holidays, or unforeseen circumstances. We will make reasonable efforts to notify you of any changes.

5. Prohibited Conduct

By using our website and services, you agree not to:

• Submit false, inaccurate, or misleading information.
• Use the services for any unauthorized or illegal purpose.
